Form 12 (Aspect Inspection Certificate, Qld)
Form 12 is the Queensland Aspect Inspection Certificate an appointed competent person (often an RPEQ) gives the certifier to confirm an aspect of work complies.

How it works:
- A building certifier can use a competent person to inspect aspects of a stage of building work, after first determining the person is competent for that aspect.
- The competent person inspects, then issues a Form 12 to the certifier certifying that aspect.
- The certifier relies on the Form 12s to be satisfied a stage is complete and compliant.
Who must sign it (single detached Class 1a examples):
- For footing reinforcement and formwork, and slab formwork and reinforcement, the competent person must be a registered professional engineer (RPEQ).
- For boundary clearance, the competent person must be a cadastral surveyor.
Form 12 vs Form 15:
- Form 15 certifies the design complies (design stage, RPEQ for engineering).
- Form 12 certifies an inspected aspect of the built work complies (construction stage, by the appointed competent person).
For a builder: line up the right competent person (often an RPEQ for structural footings and slab) to inspect and issue the Form 12 at the right stage, because the certifier needs it before the stage can be signed off. A missing Form 12 stalls the stage inspection, so book the engineer’s inspection to suit your pour and cover sequence.
